Submit a Claim / Payor ID

Claim submissions can be submitted via mail or fax. Send to Midwest Dental Benefits, 1335 Industrial Boulevard, Suite 100, Minneapolis, MN 55413 or fax to 612-767-9200.

The payor ID number for the MDB is 41101.

The National Provider Identifier (NPI) will be required for claim submission beginning in May of 2007. Upon instigation, the MDB will be utilizing the NPI for claim acceptance.
