Q. How do I submit a change in status information?

Q. How do I get a copy of the benefits summary?

A. Benefits Books are made available in PDF format. Please contact your Account Manager at 800-836-4098 for the most up to date version. If you wish to have Benefit Books printed, you can order by filling out the Order Form online or e-mailing your request to dloder@mdbtpa.com. There is a fee per booklet to cover printing and shipping costs.

Q. How do I order ID cards?

A. If you would like to order ID cards for an employee or for your entire group, please call us at 800-836-4098 or click Request ID Card to submit a form online.

Q. How will I be invoiced? When will it be available?

A. Invoices are mailed no later than the 5th business day of the month. You may elect to receive invoices by email or traditional U.S. mail.

Q. What kind of reporting is available? How do I request reports?

A. A variety of reports are available to you including enrollment, billing, and claims utilization. This includes both standard and customized reports based on your needs. Please contact your Account Manager for assistance.
