A. You can order more benefit books for your group by calling us at 800-836-4098, filling out the order form online, or e-mailing your request to dloder@mdbtpa.com. There is a minimal charge to cover printing and shipping costs.

A. If you would like to order ID cards for an employee or for your entire group, please call us at 800-836-4098 or click here to submit a form on-line.

Q. How do I submit an enrollment or change of status form?

A. You can fill out an enrollment form or change of status form on this website or you can fill out the paper form and fax to our office at 612-767-9200 or mail to Midwest Dental Benefits, 1335 Industrial Boulevard, Suite 100, Minneapolis, MN 55413.
