Q. What Dentists can I see?

A. You may see the dentist of your choice. Family members may use the same dentist or another dentist, without restriction. In addition, there are no restrictions to seeing or selecting a dental specialist. Midwest Dental Benefits does not use a "dental network". Our plans do not impose many of the restrictions or limitations that common dental plans include.

Q. What is my co-pay - (or coinsurance)?

A. Each employer group may have a unique set of benefits. Please refer to your benefit booklet for the amount or percentage that is your responsibility. You may also contact our customer service department at 800-836-4098 or go to the dental benefits summary section of this website. You will need your group number to access your information.

Q. What is covered under my plan?

A. Your benefit book gives a breakdown of what is covered and not covered with your dental plan. For a copy of your benefit book, please contact your Human Resources department.

Q. What benefits do I have remaining for this year?

A. Please give us a call at 800-836-4098 and we can provide you with an accurate summary of your account, including how much you have used and how much you have remaining.

Q. What is my group number?

A. Your group number is listed on your ID Card and in your benefit book. You may also call us at 800-836-4098 and our customer service center can provide that information.

Q. How can I add family members, change my address, etc.?

A. Please contact your human resources department to make any changes to your eligibility. To change your address, we encourage you to notify your employer, but you may also call us at 800-836-4098.

Q. What do you need for Student Verification?

A. We will accept school forms that include the name of the university or technical college and the student's name. This includes grade reports, class schedules, and tuition payment receipts. If your student's last name is different from yours, we also ask that you include your name when submitting the forms.

Q. Do I need to get a Pre-Authorization for dental treatment?

A. Midwest Dental Benefits does not requite pre-authorizations for work. If you wish to obtain an estimate of your covered benefits, you or your dental office may submit a form or call us with the appropriate procedure information and we will provide an answer promptly. (Sorry, these can be estimates only, as actual treatment or benefits remaining may have changed prior to receiving the dental claim).

Q. Can I get a copy of my Explanation of Benefits (EOB)?

A. Call our customer service center at 800-836-4098 and we can fax or mail you a copy of your EOB. You will routinely receive one approximately 2-3 weeks after a claim has been submitted for care you have received.

Q. How do I get a Benefit Book?

A. Please contact your Human Resource department to receive a copy of your benefit book.
